Architect or Contractor? The Structural Blueprint of Clinical Architecture

In my book, The Smile Owner’s Manual, I reference the architect Louis Henry Sullivan, who famously stated that “form follows function”. After more than 20 years as a dentist and my recent deep-dive into the MBA world, I have come to realize that this principle is the dividing line between commodity dentistry and Clinical Architecture.

Most people view a dentist as a “contractor,” someone they call only when a “shingle” (a filling) falls off or a “pipe” (a tooth) leaks. But a contractor’s job is to repair; while a Clinical Architect’s job is to design for endurance. Before we discuss aesthetics, we must ensure the “dental house” is built on a solid foundation.

The Blueprint: Five Cosmetic Prerequisites

To achieve a result that stands the test of time, our practice adheres to a rigorous blueprint. We do not “slap on veneers” until we have addressed the following five areas of health:

  1.   Oral Cancer Screening: We prioritize this because early detection of precursor tissue changes is a life-saving necessity.
  2.   Gum Health (The Foundation): Periodontal disease is the primary cause of tooth loss and is linked to heart disease and stroke; a beautiful smile requires a healthy foundation.
  3.   Airway and Sleep Apnea: It is nearly impossible to thrive when you stop breathing multiple times an hour. We treat the airway as a critical component of whole-body health.
  4.   Clenching and Grinding: Tooth enamel is the hardest substance in the human body, yet many patients literally wear their teeth down to the gums. We must resolve these forces before investing in restorations.
  5.   Chewing Function: Your teeth should mesh together like cogs on a gear, managing forces that can exceed 400 pounds.
